I am simulating a roof turbine ventilator using sliding mesh technique. I didn't get any error or warning during the iterations but somehow in the result my model look like this. I am hoping that someone will guide me to solve this issue.

It looks like your rotation axis for the turbine is incorrect. Instead of rotating around the Y axis it looks like it is rotating around the Z. Check how you are defining your rotation in the simulation and make sure your axis are correct.

I'm surprised this did not give an error in Fluent, so that may not be the issue.
