My fluid domain is the volume outside of a sphere (solid) that exists inside a cube (solid). The following mesh tools were used: Surface remesher, trimmer, and prism layer.

I've cut the domain in half, so the depression (bowl) is half of the spherical shell.

Please note the irregular elements of the mesh in the 6 prism layers and at the interface between the outermost prism layer and structured hexahedral elements.

What is this type of meshing error called and how can it be mitigated?

Are you viewing this in Paraview? These cells may be due to the poly decomposition in Paraview or the decomposition through exporting the mesh to another format. Do these cells show up in STAR-CCM+ natively?

Yes, I am viewing in ParaView.

Thank you for the insight regarding polyhedral decomposition!

I've learned that using the SLICE and CLIP tools is not desired. The recommended filter is 'ExtractCellsByRegion' to avoid the decompostion of polys into tets (see ref: https://openfoamwiki.net/index.php/FAQ/Postprocessing)

Using the suggested filter, I now see the following 5-faced elements in the previously mentioned prism layer of Star-CCM+
