Default 3d simulation of bubblefoam case with twophaseeulerfoam solver

Hello,

I generated my own mesh in bubblefoam case with twophaseeulerfoam solver and I wanna simulate 3 dimentional air injection in water. Main question are
1- how can I define open boundary (top)
2- How can I set proper time step
3- my results are 2d but my case specification is 3d. whats the problem?
4- whats the error with thermo.h in these cases
5- whats the reason for high courant number in this case
