Default high velocity in stagnation regions

I am running simplefoam and am getting high velocity in cavity / stagnation regions, which sometimes dissipates, but other times causes the solution to diverge.

It seems to be related to the initial conditions. If I use say U=10m/sec as the initial condition, it causes the problem. But if initialise to U=0m/sec there is no such problem.

Has anyone any idea why there is this instability in stagnation regions?
I have tried different URF settings, I wonder if its related to the solver settings. I also wonder if related to the wall turbulence value settings. Any ideas/suggestions welcome. Thanks.
