turbulence model convergence problem

Default turbulence model convergence problem

I am running simpleFoam on a turbulent model with k-epsilon method and am getting the following messages and I can't figure out what it's telling me.

I'm not sure if I have a mesh problem or if my convergence settings are too restrictive.
what surprised me was that the convergence was proceeding smoothly until it suddenly diverged within a few cycles.

Linux Ubuntu 16.04
OpenFOAM 4.1
