Simple foam error

Exclamation Simple foam error

Hi guys,
i tried to run a simulation with simpleFoam , but during the solving process i got this Message in time step 40!
it's a flow in a square tube and flow is laminar !
what does it mean and what's wrong with my case?

Dear Mohsen

number of iteration is around 1000, final residual is around 1E+6, so your problem is diverging.
1- check your BC
2- check initial condition
