I struggle with the error messages that OpenFOAM gives. I am not a programmer, and sometimes the way things are stated do not give me much hope of finding solutions.

Using a search engine for:

a couple of things pop up that may be relevant:

SnappyHexMesh error: point is not inside mesh, face, or edge

Maybe check the volume where you are refining your mesh, and confirm details around where you are refining the mesh.

It would also be worth looking at details and internet searches for snappyHexMesh instead of just OpenFOAM and the error message generated. I don't use snappyHexMesh, so am not helpful here.
