Default fluentMeshToFoam(ICEM) - bad stream error

I use ICEM to generate 3D mesh (cylinder.msh file) and convert it into OpenFOAM, but I met some errors:

(i) when using the commend

fluentMeshToFoam cylinder.msh
, the error is:
Code:
--> FOAM FATAL IO ERROR: 
Attempt to get back from bad stream

file: IStringStream.sourceFile at line 1.

    From function bool Foam::Istream::getBack(Foam::token&)
    in file db/IOstreams/IOstreams/Istream.C at line 56.

FOAM exiting
(ii) when using the commend
Code:
fluent3DMeshToFoam cylinder.msh
, the process will be quickly completed but all the files in PolyMesh folder are 0, like 0 points, 0 faces, ..

I don't know how to solve this problem. Is there something wrong when generating mesh in ICEM? Thanks!
