Default tetra shell meshing ORFN part

hello
I tried to mesh a very bad geomery (Patch independent). I get ORFN part generated by default. I know its region out of domain but what is the reason that it gets generated. It might be a simple thing but I could not work out. if any one could help me understand. why does it not happen with patch dependent meshing.
