Grid generation for artery flow domain

In general, which kind of grid (structured/ unstructured, hexahedral/tetrahedral, etc) is used in Coronary artery flow simulation with time-dependent moving boundaries? Is there any special commercial code to generate it, or any commercial CFD code can implement it?
