Default Simulation Volume Questions - Size/Distance from model

Pardon My ignorance, but I am looking to start off on the right foot with my series of simulations.

What is the standard distance that most meshes are extended upstream (how many calibers), around the model, and downstream?

Are there any metrics that are generally used, such as vorticity, to determine if the volume was too small or too close to the model?

I am hungry for information, if you have and references that you recommend, I'll gladly check them out.
