Fifth-order triangular element

Default Fifth-order triangular element

Can anyone point me to the formula for the fifth-order complete finite element on a unit right-angle triangle? This element has function and first and second derivatives at the corner nodes plus some internal nodes. Mitchel and Wait give a reduced form with the internal nodal degrees-of-freedom eliminated, but not the complete fifth-order element, saying that this element is "useless."
