Difficulty initializing 3D detonation simulation

Hi,
I'm struggling with initializing a detonation in a 3D computational simulation. My simulation is set up within a 3D cylindrical domain (Figure-1) where the inflow and outflow are indicated by red arrows. I've included a hand-drawn sketch of my boundary conditions in Figure-2. While using the same boundary conditions, I attempted a 2D simulation (Figure-3) which worked adequately and produced a detonation wave. For reference, I'm providing a link to the paper I used as a reference and also highlighting the inlet boundary condition section in Figure-4 of that paper. Could you please provide some insight into why I might be having trouble initializing my 3D simulation? Reference paper: Schwer, D., & Kailasanath, K. (2011).
