It is very hard to tell from your description.
Often the problem is how you have drawed your geometry. You can maybe try to simplify your geometry and mesh and see if that works.
The auto generation tool/Check mesh/repair works very well. You can take a look at the tutorial file of ICEM and follow the user guide. Ofcause this will only work if you are working in ICEM.
