Post getting error that amg-divergence detected in x momentum

I am working on a multiphase VOF modelin steady state
It is kind of tank drainage with one inlet and 6 outlets.
I am done with everything till Adapt-> Region and Solution Initialization -> Patch .
This two things I skipped
Is there significance of these steps if any please tell me what to do.
I calculated solution by first order of both momentum and pressure and I am getting some result but for more accuracy when I go for second order of any of two I get error that amg-divergence detected in x momentum . I decreased under relaxation factor of momentum to 0.1 still I am getting error in second order calculations.


Please help.
