Default 2 way FSI simulation errors

I am running the simulation on the 3D FSI problem, and got the following errors:'ANSYS Solver terminated with return code 256'.
The interface are around a deformabel plate and the fluid drives the plate to deform and of course the mesh deformation reacts on the fluid flow.
Does anyone have similar experience on this issue? Any reference suggested on the CFX two way FSI simulation? I have read the only one I can get, that is the tutorial for "ANSYS Solver terminated with return code 256". I think I may need to see how other do it.

The error messages in the CFX out file show that the Mechanical (ANSYS) solver failed. You'll need to look in the ANSYS.stdout file to see why.
