Default IECM CFD prism mesh question!!!

hi, everyone. i am a new user to IECM CFD. I'm using it to mesh a butterfly valve. but many times, it can't generate "prism mesh" (sometimes it can). i don't know why! some people told me it's about my quality of "tet mesh", but after i improved it, i still can't be done. is there anybody know the reason? help me, please.

**************here is the error message******************

internal error in handle_tet

Assertion failed: stop_columns == 0, file stick.C, line 178
