Hexa core - Error

An error was encountered when I tried to generate a hexa core mesh out of an existing tet prism mesh. The details can be found in the word document attached to this post
Some one please go through it and suggest a solution , any help is greatly appreciated

It doesn't like your mesh size in the volume... The ICEM CFD HexCore is limited by rules that require conformal mesh between the hexas and the tetras... If you have any hanging nodes their is a failure... If your mesh size (between the tetra and Hexas) is too far off, you get a failure...
