Point Insertion During Mesh Generation

This is a more generic mesh generation question but I would like to get some insight from the community on that.
How does a mesh generator decide how and where it will create points to disretize the edges/surfaces/volumes of a given domain, based on the user specifications that we set before creating the mesh (e.g. "maximum size" in ICEM)? To elaborate more, we provide, through CAD, points, curves, surfaces that represent our geometry. From there, and after having specified some parameters like minimum edge length, growth ratio etc, are there specific techniques or algorithms that mesh generators use to decide on the the distribution of points in the 2D/3D space, their exact number, their position and so on?
