Default Leakage in volume meshing

I am trying to create volume mesh with delaunay method, however i get leakage error from one domain to another. I applied check mesh options, sorted errors and possible problems in subsets, and corrected all the errors. However still having the error. Does anyone have an idea what might cause this problem?
