[ICEM] Problems with meshing in ICEM CFD

Hello,
I am trying to mesh some kind of a drilling whole with ICEM CFD (have a look at the picture). I think my blocking should be ok. I created an o-grid to get better angles... As you can see there is some problem with the pre-mesh. The elements at the surface are not generated correctly. It seem as if there is some kind of wrong association between the edges. There is another strange thing: When I generate a scan plane, I can see that the mesh looks better the closer the plane is placed to the end of the drilling whole. At the end of the drilling whole the mesh looks good!! So, the origin of the problem seems to be at the surface. I checked almost everything. Does anyone know what the problem is?
